Every AI agent looks brilliant in the demo. Clean input, a cooperative tester, a scripted path where the agent's strengths are on display and its weak spots are conveniently off-screen. Then it meets real users, real data, and a real workflow — and it falls apart. Not loudly. Quietly. Which is worse.

The real reason: failure compounds

The single most important thing to understand about agents is that errors don't stay isolated — they multiply across steps.

Say your agent is 85% reliable at each individual step. That sounds solid. But a real task isn't one step; it's a chain. For an eight-step workflow, the odds of getting the whole thing right are 0.85 to the eighth power — about 27%.

Read that again. An agent that's "85% reliable" completes a full eight-step task correctly roughly one time in four. Add more steps and it gets worse, fast. This is why agents ace the three-step demo and choke on the realistic ten-step job: the demo never had enough steps for the compounding to show.

Most teams discover this the hard way — after launch, when the failure rate is a production incident instead of a line on a slide.

The failures are silent

Here's the part that makes it dangerous: agents don't fail like normal software.

Normal software crashes. It throws an error. A monitor lights up red and someone gets paged. You know.

An agent doesn't do you that favor. It quietly does 80% of a task, hallucinates a "done," and reports success. Your server is healthy. Your latency looks fine. Every dashboard is green — while the agent is confidently returning garbage. You find out it lied when the missing 20% turns out to be the part that mattered.

Traditional monitoring can't catch this, because traditional monitoring asks "is it up?" The right question for an AI system is "is it still correct?" — and that's a completely different instrument.

The four places agents actually break

When an agent fails in production, it almost always traces back to one of four areas. If you want to know whether yours is ready, these are the places to look.

1. Architecture

Most agent failures aren't model failures — they're plumbing failures. State that gets overwritten, steps with undefined inputs and outputs, no checkpoints to recover from, a context window rebuilt from whatever happened to be lying around. If your agent can't restart from a known-good state, it can't be trusted at scale.

2. Memory and context

Watch a long run closely and you'll notice the context slowly filling up — not with your goal, but with the agent's own earlier output, including its earlier mistakes. By step six or seven it's mostly reacting to itself. Call it Context Bleed: the agent drifts because its working memory has quietly become a record of its own confusion. Unmanaged memory is one of the most common silent killers.

3. Failure handling

Ask a hard question of any agent system: what happens when a step fails? If the answer is "it logs it and keeps going," you don't have failure handling — you have a way to keep producing wrong answers politely. Real systems define a response for every failure type, cap their retries, halt on cascades, and keep a rollback plan that's actually been tested.

4. Human oversight

Full autonomy on low-stakes work is fine. Full autonomy on the step that moves the money, sends the email, or deletes the record is how you get the horror stories. Reliable systems decide in advance which actions need a human in the loop — and build the checkpoint before the action, not the post-mortem after it.

How to know if yours is ready

You can't eyeball this. "It worked in testing" is the exact trap — testing is the demo with better lighting. Readiness isn't a feeling; it's the result of running your system, deliberately, against each of the four dimensions above and seeing where it actually stands.

That means going check by check. Is every step's input and output defined? Is your canonical log append-only? Does a below-confidence output trigger a human review, or a silent override? Does a cascade of failures halt the system, or quietly compound? Each honest answer is a point for or against shipping.
